Overview

This Comprehensive Mass General Brigham Heart Failure & Shock symposium will provide practical insights for all clinicians who manage patients with heart failure (HF) and cardiogenic shock. This offering is a two-part course, of which participants can choose to partake in one or both parts. The first day of this three-day course will be an in-depth review of cardiogenic shock while the next two days focus on the latest HF innovations with an emphasis on the multidisciplinary approach to treating HF patients. 

For the Shock course, expert speakers will address state-of-the-art issues including: up-to-date knowledge of cardiogenic shock pathophysiology, shock phenotypes, and trajectories; best practices in shock diagnosis and management; the roles of temporary mechanical circulatory support, and challenging case discussions. 

For the HF course, leaders in the field will discuss: general HF management across the spectrum of EF, pathology of the failing heart, the fifth pillar - when medications are not enough, HFpEF GDMT in 2027, special cardiomyopathies, and genetic testing (valvular, hypertrophic, inflammatory, amyloid, and cancer-induced). There will be sessions on navigating special issues in the context of HF, such as pregnancy, obesity, and palliative care referral. 

The symposium will illustrate the Mass General Brigham Heart Failure Section's comprehensive approach to the management of heart failure and cardiogenic shock for all cardiovascular clinicians. 

Learning Objectives

  • Critically evaluate current knowledge gaps in cardiogenic shock and assess the design, objectives, and potential clinical implications of emerging and upcoming clinical trials.
  • Summarize the mechanisms and indications of vasoactive drugs used in cardiogenic shock.
  • Describe the indications and managements of temporary mechanical circulatory support, including VA-ECMO, and transitioning to durable LVAD or transplant.
  • Explain the role of palliative care in the comprehensive management of patients with shock, including symptom management, goals-of-care discussions, and alignment of treatment with patient-centered outcomes.
  • Explain the pathophysiology of HF and describe the diagnostic evaluation of a patient presenting with HF.
  • Describe current evidence-based medical and device therapeutics for HFrEF and HFpEF.
  • Describe the diagnostic and therapeutic approaches for special cardiomyopathies (valvular, amyloid, hypertrophic, and cancer-related).
  • Describe the clinical indications for cardiopulmonary exercise stress testing and interpret key findings to guide diagnosis, prognosis, and management decisions.
  • Summarize the management of arrhythmias, right heart failure, and obesity.
  • Summarize the role of artificial intelligence in HF management.
